Medicare Patients With Chronic Conditions Can Now Get No-Cost Support Between Doctor Visits

Original Medicare patients in Massachusetts can now get regular check-ins between doctor visits, at no cost, through Medicare’s ACCESS Model.

MDinteractive Health wants to change that. As a participant in Medicare’s new ACCESS Model, it now offers Original Medicare patients in Massachusetts steady, personal support between visits — at no cost to them — so that keeping up with a chronic condition feels less like doing it alone.

At the center is Sarah, MDinteractive Health’s AI care assistant, who checks in with patients by phone between appointments. She asks how they’re doing, helps them stay on track with their medications and readings, shares guidance for their condition, and keeps their own doctor informed. It’s a warm, regular check-in — not a scary medical call — and if something seems off, their doctor hears about it.

Patients whose care plan includes blood pressure monitoring also get an easy-to-use home monitor at no cost, as part of their care. It sends readings to their care team automatically — no wifi, no smartphone, and no setup — so a patient can simply put on the cuff and go about their day.

The program is for people in Massachusetts with Original Medicare (Parts A and B) who are managing chronic conditions such as high blood pressure, high cholesterol, prediabetes, diabetes, kidney disease, heart disease, chronic pain, or depression or anxiety. Taking part is voluntary, patients keep every one of their own doctors, and it does not change their Medicare coverage or benefits. This is added support — not a replacement for the care they already have.

There is no cost to eligible patients. Medicare pays MDinteractive Health directly, and MDinteractive Health has chosen to waive the small monthly cost-sharing patients might otherwise pay — so patients pay nothing.
